Here's the details:
- Purchase your Amope gel Activ insoles or inserts (up to $10.00) between 9-25 and 10-23.
- Head over HERE and print your rebate form (available starting 9-25).
- Mail in form, cash register receipt and upc from package to address on form.
- Requests must be postmarked no later than 10-30.
- Valid only on the Activ insoles or inserts
- Limit 1 rebate per household.
